Primary Care Physicians (PCP) and the CATCH social worker visit the patient’s home to review medical objectives and other social and environmental factors that impede progress toward achieving PCP recommended medical objectives. Spending time with your PCP in your home allows for each party to better understand one another with the aim of improving well-being. Home visits are not intended to replace regular clinic appointments.
